29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:  On or about January 6, 2025, at a jobsite located at 4333 W Wisconsin Ave, Appleton, Wisconsin: the employer did not ensure an employee working in a trench excavation deeper than five feet was protected from cave-in by an adequate protective system.  PTS Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1682834, citation number 2,  item number 1, and was affirmed as a final order on September 29, 2023, with respect to a workplace located at the intersection of Manitowoc Road and Oneida Street, Menasha, Wisconsin.
