The employees were using a four part alloy chain sling to lift the part and the employer did not ensure that the safety latch was in place.

29 CFR 1910.184(d): Each day before being used, the sling and all fastenings and attachments were not being inspected for damage or defects by a competent person designated by the employer: On or about September 20, 2021, employees were exposed to a struck by hazard while using a 2 ton R&M Overhead Crane to move a buggy from the floor onto a pallet. The employees were using a four part alloy chain sling to lift the part and the employer did not ensure that the sling had been inspected for damage or defects before it was used that day.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 1. On or about September 27, 2021, the employer failed to ensure that effective machine guarding was in place on the Accurpress Brake 7060 to protect employees from the pinch point of the upper die contacting the bottom die during operation. 2. On or about September 27, 2021, the employer failed to ensure that effective machine guarding was in place on the Piranha Iron Worker to protect employees from the pinch point of the upper die contacting the bottom die during operation.
