29 CFR 1926.651(k)(1):Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: a) Jobsite, along 2891 Fairway Drive, Altoona, PA: On or about September 24, 2021 an employee was working on a new sewer line in an unprotected 80' long, 4' wide and 7.5' deep excavation and had not performed a daily inspection to detect potential dangers such as but not limited to; installed trench box not in the work area, an exposed and unsupported utility line running alongside of the trench wall, no ladder for safe egress.
