29 CFR 1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  Jobsite - On or about July 28, 2021, employees working to clean up debris on the ground were not protected from head injury where there was roofing activities occurring above them and they were not utilizing protective helmets.

29 CFR  1926.552(a)(1): The employer did not comply with manufacturer's specifications and limitations applicable to the operation of all hoists and elevators:   Site - On or about September 28, 2021, the employer did not comply with manufacturer specifications when assembling the RGC material hoist being used onsite where there were bolts missing from the splice plates joining the two pieces of track together.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a.) Jobsite - On September 28, 2021, employees working on stripping the back portion of the main building roof were not protected from a fall of approximately 24 feet from the eave and 28 feet from the peak where fall protection was not utilized.  b.) Jobsite - On September 29, 2021, following a closing conference on September 28, 2021 where the employer was informed of the requirement for fall protection, employees were working on an additional roof at the back of the building and were not protected from a fall of approximately 10 feet where fall protection was again not utilized.
