29 CFR  1910.147(c)(4)(i):  Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a) The Greensboro NDC: On or about September 27, 2021, the employer exposed employees to amputation hazards in that the employer failed to ensure employees utilized lockout tagout procedures while engaged in clearing jams on Alpha 11.  b) The Greensboro NDC: On or about November 26, 2021, the employer exposed employees to amputation hazards in that the employer failed to ensure employees utilized lockout tagout procedures while engaged in reinstalling Alpha 11 and 12 belts.

29 CFR  1910.335(a)(1)(i): Employees were not wearing protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed when working in an area where there was a potential electrical hazard.      The Greensboro NDC: On or before September 27, 2021, Electronic Technicians and Machine Processing Mechanics were working on live electrical parts while trouble shooting on mail processing equipment. Employees were exposed to voltages up to 480v and were not provided with appropriate electrical protective equipment.

29 CFR  1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees.    The Greensboro NDC: On or about September 27, 2021, and at times prior thereto, the employer exposed employees engaged in clearing jams to amputation hazards in that the employer failed to provide training on the recognition and magnitude of hazardous energy sources and the means and methods in which to isolate these energy sources.  The U.S. Postal Service. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(7)(i), which was contained in OSHA inspection number 1331751, citation number 1, item number 1a and was affirmed as a final order on July 19, 2019, with respect to a workplace located at 17 S. Commerce Way, Bethlehem, PA, 18017.

29 CFR  1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks:     a) The Greensboro NDC: On or about September 27, 2021, the employer failed to ensure conveyor Alpha 12 was guarded, exposing an employee to an amputation hazard.  b) The Greensboro NDC: On or about September 24, 2021, the employer failed to ensure conveyors at Alpha 14, 18, 25, 26 were guarded, exposing an employee to an amputation hazard.  c) The Greensboro NDC: On or about December 10, 2021, the employer failed to ensure conveyor at H-42 was guarded, exposing an employee to an amputation hazard.  d) The Greensboro NDC: On or about January 11, 2022, , the employer failed to ensure conveyors at E-4, A10, A-12, A-14, A-20, A-26, G-4, SP-02 were guarded, exposing an employee to an amputation hazard.  e) The Greensboro NDC: On or about January 24, 2022, employees were exposed to amputation hazards while engaged in 4 hour mail searches at 4002, 4003, H-44 and DS-1.  The U.S. Postal Service. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(2), which was contained in OSHA inspection number 1203272, citation number 1, item number 1 and was affirmed as a final order on March 20, 2017, with respect to a workplace located at 660 Westfield Road, San Francisco, CA 94128.
