29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: a) In the Consumer Fresh Produce, Inc. Mechanical Room, located at 1 21st Street, Pittsburgh, PA 15222, on or about September 28, 2021 - The lead Service Technician was using a full facepiece air purifying respirator while attempting to remove RP-1 from the refrigeration system. The lead Service Technician did not receive an annual fit test with that respirator. b) In the Consumer Fresh Produce, Inc. Mechanical Room, located at 1 21st Street, Pittsburgh, PA 15222, on or about September 28, 2021 - The second Service Technician was using a full facepiece air purifying respirator while attempting to remove RP-1 from the refrigeration system. The second Service Technician did not receive an annual fit test with that respirator.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) In the Consumer Fresh Produce, Inc. Mechanical Room, located at 1 21st Street, Pittsburgh, PA 15222, on or about September 28, 2021 - Section 4. Purging of Piping, Valves, or Components of the employer's Guidelines for Handling Anhydrous Ammonia failed to clearly identify all of the specific steps to be followed by employees to isolate RP-1 and surrounding piping, relieve hazardous residual chemical energy in RP-1 and surrounding piping, and verify the effectiveness of the energy control measures of RP-1 and surrounding piping in order to safely de-energize RP-1 and control the hazardous chemical energy exposing employees to a release of anhydrous ammonia while attempting to remove RP-1 from the refrigeration system.
