29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  Site - On or about September 22 , 2021, employees were exposed to head injuries from falling materials and debris while working directly underneath the roof where head protection was not provided or utilized to prevent the hazard.

29 CFR  1926.502(d)(17):The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  Site - On or about September 22, 2021, the employer did not ensure employees working on the roof had their lifelines attached at the center of the employees' backs. The employees had lifelines attached to the harness located on the wearer's front side.

29 CFR  1926.503(a)(2):The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  Site - On or about July 21, 2021, the employer failed to provide fall protection training to employees by a competent person to ensure that the employees understood the hazards associated with falls and the proper means necessary to minimize or eliminate these hazards.
