29 CFR  1926.20(b)(2):  It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part; such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On September 29, 2021 at 811 E Murdock Avenue in Oshkosh Wisconsin, the employer did not ensure that a competent person was performing frequent and regular inspections that could have recognized fall from elevation hazards, electrical hazards, and struck-by hazards associated with employees engaged in roofing activities.

On September 29, 2021 at 811 E Murdock Avenue in Oshkosh Wisconsin, Wisconsin, employees were operating pneumatic nail guns and battery powered wrench and drill tools without wearing safety glasses.

29 CFR  1926.405(a)(2)(ii)(I):  Flexible cords and cables shall be protected from damage.  Sharp corners and projections shall be avoided.  Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.   On September 29, 2021 at 811 E Murdock Avenue in Oshkosh Wisconsin, employees were using two unprotected extension cords that were pinched in a metal doorway frame.  The cords were plugged into an indoor receptacle, positioned through the doorway with a self-closing door, and used to energize equipment on a roof surface.

29 CFR  1926.1053(b)(1):  When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.  On September 29, 2021 at 811 E Murdock Avenue in Oshkosh Wisconsin, employees were using an extension ladder to access/egress a roof surface 21.5 feet above surface level without the ladder side rails extended at least 3 feet above the roof surface.  The ladder side rails extended 15 inches above the roof surface.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems.  On September 29, 2021 at 811 E Murdock Avenue in Oshkosh Wisconsin, employees were working on a 5:12 pitched roof surface 21.5 feet above surface level without utilizing conventional fall protection equipment.
