On or about September 29, 2021, Employees were exposed to struck-by hazard in the eye while using a pneumatic nail gun, on a construction site in Fisher, Illinois, and the employer did not ensure the use of eye protection.

On or about September 29, 2021, Employees were exposed to a fall hazard of approximately 12ft. on a construction site in Fisher, Illinois, and the employer did not ensure an adequate fall protection system was being used.
