29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) On July 30, 2021, at the above addressed jobsite, employees were exposed to eye injury while operating pneumatic nail guns without eye protection while installing a new roof on a commercial property.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.  AM Restoration Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.102(a)(1), which was contained in OSHA inspection number 1409510, citation number 2, item number 2 and was affirmed as a final order on November 19, 2019, with respect to a workplace located at 3712 Celeste Lane in Naperville, Illinois.  AM Restoration Services, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1302488, citation number 1, item number 2 and was affirmed as a final order on April 19, 2018, with respect to a workplace located at 546 Marion Street in Oak Park, Illinois.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) On July 30, 2021, at the above addressed jobsite, employees conducting installation of a roof on a new construction commercial property, at approximately fifteen (15') feet nine (9) inches above the ground, were not provided personal fall arrest systems or other means of fall protection, thereby exposing the employee to fall hazards.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
