29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) On September 15, 2022, at the above addressed jobsite, employees performing roofing work were exposed to fall hazards of approximately 22 feet.  b) On September 16, 2022, at the above addressed jobsite, employees performing roofing work were exposed to fall hazards of approximately 22 feet.  c) On September 16, 2022, at the above addressed jobsite, employees performing roofing work were exposed to fall hazards of approximately 9 feet 10 inches.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  a) On September 15, 2022, at the above addressed jobsite, employees were exposed to eye injury while operating pneumatic nail guns without eye protection while installing a new roof on residential townhomes.    b) On September 16, 2022, at the above addressed jobsite, employees were exposed to eye injury while operating pneumatic nail guns without eye protection while installing a new roof on residential townhomes.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. AM Restoration Services, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1302488, citation number 1, item number 2, and was affirmed as a final order on April 19, 2018, with respect to a workplace located at 546 Marion Street in Oak Park, Illinois.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On September 15, 2022, at the above addressed jobsite, portable ladders used by employees to access and egress the roof, measuring approximately 9 feet from ground to eve, were not extended to have the side rails extend at least three (3) feet above the roofline.    b) On September 16, 2022, at the above addressed jobsite, portable ladders used by employees to access and egress the roof, measuring approximately 9 feet 10 inches from ground to eve, were not extended to have the side rails extend at least three (3) feet above the roofline.    No abatement certification or documentation is required for this item.  AM Restoration Services, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1582757, citation number 2, item number 1, and was affirmed as a final order on September 7, 2022, with respect to a workplace located at 501 Prairie Avenue, Wheaton, Illinois.
