OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:   (a)  On or about October 1, 2021, and at times prior, the employer did not ensure that employees were safe from crushing and struck-by hazards in that the following lifts were noted with deficiencies:     (1)  Rotary Lift, model SPOA12N410, serial #AUI0310117, located in bay 4, did not have the locking mechanism locked  into place while the              lift was raised and in use.     (2)  In ground hydraulic Rotary Lift, model SI210, serial number not on lift, located in bay 11, did not have the locking mechanism locked              into place while the lift was raised and in use.      Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
