29 CFR 1910.178(l)(4)(ii)(A):1910.178(l)(4)(ii)(A): Refresher training in relevant topics was not provided to operator(s) when the operator was observed operating the vehicle in an unsafe manner: a) Facility - On or about September 20, 2021, an employee operating a forklift was not provided refresher training after this employee was observed to be operating the forklift in an unsafe manner when he damaged a machine while operating a forklift. b) Facility - On or about September 29, 2021, an employee operating a forklift was not provided refresher training after this employee was observed to be operating the forklift in an unsafe manner when he was carrying 3 units of material with the forklift and went around a turn and the units then fell over striking another worker walking in the safety walkway. The company's policy is to not lift more than 2 units of material with the forklifts.
