29 CFR 1926.451(g)(1)(i):Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) Ridge Stone General Contractors on or about October 04, 2021, at the site location of 7218 Lighthouse Way Perrysburg, OH; The employer in control of the job site construction activities (as prime contractor) failed to ensure subcontracted employees were adequately protected from falls. These employees, engaged in siding activities from a ladder jack scaffold were exposed to falls of 14.6 feet to the ground below.
