29 CFR 1926.451(b)(1)(ii): The platform was not planked or decked as fully as possible and the remaining open space between the platform and the uprights exceeded 9.5 inches (24.1 cm). On or about October 4, 2021, on the Northside of the site employees were performing insulation work on a scaffold that was not fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffolds, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. On or about October 4, 2021, on the south side of the Jobsite, employees were performing insulation tasks on a scaffold that did have proper access or egress where the scaffold platforms were greater than two (2) feet from the lower level.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about October 4, 2021, on the south side of the project site; employees were on scaffolding with no guardrail system installed while performing insulation work on the exterior of the building at heights greater than ten (10) feet.
