29 CFR  1910.147(c)(4)(ii):Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  a) Walnut Creek Lumber Company, Ltd located at 10433 Pleasant Hill Road Northwest Dundee, OH 44624:  On and preceding October 4, 2021, the employer did not ensure that machine specific lockout procedures, as described and required by 1910.147(c)(4)(ii)(A) through (D) were developed and written for machines such as, but not limited to a Planer, Chop saw and Mereen Johnson Arbor Gang Rip saw.   The following condition exposed employees to caught-in and struck-by hazards.

29 CFR  1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:   a) Walnut Creek Lumber Company, Ltd located at 10433 Pleasant Hill Road Northwest Dundee, OH 44624:  On and preceding October 4, 2021, the employer did not conduct annual or more frequent inspections of the energy control procedures to ensure that the procedures and requirements of this standard were followed.  Application of hazardous energy control was applied to machines such as, but not limited to a Planer, Chop saw and Mereen Johnson Arbor Gang Rip saw.  Inspections of the program, procedures and application of hazardous energy control were not being conducted at least annually.   The following condition exposed employees to caught-in and struck-by hazards.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a) Walnut Creek Lumber Company, Ltd located at 10433 Pleasant Hill Road Northwest Dundee, OH 44624:  On and preceding October 4, 2021, the employer did not ensure that ingoing nip points were protected on a transfer belt/pulley which was positioned on the back side of a camera scanner used as a part of the conveyor system in the rough mill.  The following condition exposed employees to caught-in hazards.

29 CFR  1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:   a) Walnut Creek Lumber Company, Ltd located at 10433 Pleasant Hill Road Northwest Dundee, OH 44624:  On and preceding October 4, 2021, the employer did not ensure that ingoing nip points were protected on a large chain and sprocket conveyor referred to as the transfer deck.  Employees working in the area were exposed to caught-in hazards in the area of two outboard chains/sprockets which were used to transfer sections of wood.
