OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and/or crushed-by hazards: (a) On or about 09/27/2021 - Plate Yard Area, 20-Ton, Deshazo overhead crane, eight rectangular steel plates weighing approximately 1000 pounds each were being lifted from the ground exposing employees to struck-by and/or crushed-by hazards when the plate hooks slid off the plates.
