29 CFR  1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.   a)  On or about 10/01/2021, at the village of Hilton Department of Public Works, located on 50 Henry Street, in Hilton, New York, the employer did not instruct employees operating a mobile elevating work platform (scissor lift - Skyjack Model SJIII 3226) in the recognition of unsafe work conditions and control of hazards during the installation of security cameras, such as, but not limited to: struck by hazards of garage door and unexpected movement of equipment.   An employee fell 12 feet to ground level when the scissor lift was struck by a large overhead door (Hilton Department of Public Works) that was unexpectedly opened.  NO ABATEMENT DOCUMENTATION REQUIRED
