29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  On or about September 25, 2021, employees working with the Dyna-Bal D40 Balance Table were exposed to machine hazards associated with moving parts when wheels mounted on the rotating shaft of the machine were not guarded to prevent employee exposure to the entanglement and caught-n hazards.

29 CFR  1910.219(c)(2)(i):All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.  On or about September 25, 2021, employees were exposed to entanglement hazards when the rotating shafts on the Dyna-Bal D40 were not guarded to prevent contact with the rotating parts.

29 CFR  1910.219(e)(3)(i): Vertical and inclined belts shall be enclosed by a guard conforming to standards in paragraphs (m) and (o) of this section.  On or about September 25, 2021, employees were exposed to the potential caught-in hazards when the vertical drive belts for the Dyna-Bal D40 were not enclosed to prevent exposure.

29 CFR  1910.219(f)(3):Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.  On or about September 25, 2021, employees were exposed to the in-running nip points of the chain and sprocket drives on the Sterling Electric motor located on backside of the Dyna-Bal D40 Balance Table.

29 CFR  1910.334(a)(2)(ii):If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made.  On or about October 5, 2021, employees were exposed to potential shock hazards when: a) working with a damaged extension cord that had received a cut to the protective insulation that left the interior wires exposed. The cable had been wrapped in electrical tape but the tape did not prevent the exposure of the wire or provide adequate protection against shock.  b) working with a Metabo W11-125 Quick angle grinder whose power cable had been damaged. The cable had been wrapped in electrical tape but the tape did not provide adequate protection against shock.
