OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards from falling loads and/or automotive lift parts: a. Site- On or about September 27, 2021 an employee was struck and crushed by a GMC 2500HD truck that had fallen off of a Forward, model DP10AN200M, light duty two post automotive lift where the required adapters needed for engaging a pickup truck frame were not used to lift the vehicle. b. Site- On or about September 27, 2021 an employee was struck and crushed by a GMC 2500HD truck that had fallen off of a Forward, model DP10AN200M, light duty two post automotive lift; where the vehicle was lifted without using the manufacturer's recommended lift points or the lift points illustrated in the ALI/LP Guide, Vehicle Lifting Points/Quick Reference Guide for Frame Engaging Lifts at all times. Prior to the vehicle falling off of the automotive lift the vehicle was lifted two times using two different lift points on the rear of the vehicle with the rear swing arms of the automotive lift. c. Site- On or about September 27, 2021 an employee was struck and crushed by a GMC 2500HD truck that had fallen off of a Forward, model DP10AN200M, light duty two post automotive lift where the lift had not received an annual inspection from a qualified inspector, as required by ANSI/ALI ALOIM-2008.
