WAREHOUSE: On or about October 5, 2021, the employer did not provide an appropriate extension ladder to transfer two fans from the forks of the GV 50 HD powered industrial truck (PIT) to the racks, which resulted in a worker falling approximately fourteen (14') feet backwards resulting in fractures and bruises to the back and head of the worker. ABATED DURING THE INSPECTION.
