A) Nickolas Staffing, LLC dba FMT, 2000 Industrial Drive, Findlay, Ohio 45840: On or about September 28, 2021, the employer did not ensure employees were trained in the proper care, inspection, storage, and use of fiberglass extension ladders. Untrained employee(s) operating from the ladder in this condition were exposed to fall hazards.

29 CFR 1904.29(b)(1): The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300: A) Nickolas Staffing, LLC dba FMT, 2000 Industrial Drive, Findlay, Ohio 45840: On or about September 28, 2021, the employer failed to ensure that the OSHA Form 300 for calendar year 2021 contained a complete description in column F. The 2021 300 log did not define the object that directly caused the injury to the employee.
