(a) At the worksite:  On or about 10/1/2021, employees were not protected from overhead hazards such as falling and flying debris, while working below roofers.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  (a) On the roof of the residence:  On or about 10/1/2021, employees were not provided with fall protection training, while exposed to fall hazards of up to approximately 15 feet in height during a residential roofing project.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  (a) At the front of the residence:  On or about 10/1/2021, employees were exposed to fall hazards of approximately 11 feet 9 inches in height, while ascending and descending ladders that did not extend at least 3 feet above the upper landing surface.

(a) On the  roof of the residence:  On or about 10/1/2021, employees were exposed to fall hazards of up to approximately 15 feet in height, while working on a sloped residential roof without any means of fall protection.
