29 CFR 1910.106(d)(3)(ii)(b): The bottom, sides, and/or tops of wooden cabinets were not constructed of an approved grade of plywood at least 1 inch in thickness, that would prevent it from breaking down or delaminating under fire conditions: a) 3201 Grand Avenue, Billings, MT: On or about October 10, 2021, and at times prior, employees were exposed to fire hazards while loading and unloading wood stock off of delivery trucks and when servicing the forklifts in direct proximity of the flammable liquids storage which includes chemicals including, but not limited to; Motor Fuel (flash point: -45 degrees F (-42.8 C). The flammable liquids were stored in an exposed wooden box with holes and broken pieces of plywood.

29 CFR 1910.110(c)(4)(ii): LP-Gas containers were not set upon a firm foundation or otherwise firmly secured: a) 3201 Grand Ave, Billings, MT: On or about October 7, 2021, and at times prior, employees were exposed to fire and secondary explosion hazards while working with or around a large LP Gas tank that was not firmly affixed to the ground. The LP Gas Tank was supported by multiple concrete cinder blocks that were not seated stably on the ground.
