29 CFR  1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:  Split Steel Cell - On or about October 7, 2021, the energy control procedures for the OJIM splitter machines did not clearly and specifically outline the steps for use and placement of wood blocking to isolate stored energy in the upper ram assembly.

29 CFR  1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  a.) Split Steel Cell - On or about October 7, 2021, the OJIM splitter operators were not utilizing energy isolating devices to isolate the machines from electrical, pneumatic, hydraulic and stored energy sources during tool changes where operators were reaching into energized machines to perform this task.  b.) Split Steel Cell, OJIM Splitter Machine SP-011 - On or about October 5, 2021, maintenance employees used a wooden block inside the machine to isolate the stored energy in the upper ram assembly that was not located and secured in a manner to isolate the energy source.

29 CFR  1910.147(e)(1): Before lockout tagout devices were removed, the work area was not inspected to ensure that nonessential items had been removed or ensured that machine or equipment components were operationally intact.  Split Steel Cell, OJIM splitter machine SP-011 - On or about October 5, 2021, the work area was not inspected by maintenance employees to ensure that the machine or equipment components were operationally intact. A wooden block used by maintenance employees as an energy isolating device was not removed from the machine prior to removal of the lockout devices and giving control of the machine to the operator.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Split Steel Cell, OJIM splitter machines - On or about October 7, 2021, the mag separators that were part of the coolant recovery system were left unguarded on all seven machines where employees are exposed to rotating parts and ingoing nip points.  NHI Mechanical Motion DBA New Hampshire Industries was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1509705, citation number 1, item number 1 and was affirmed as a final order on April 26, 2021, with respect to a workplace located at 35 Connecticut Riverbend Place, Claremont, NH 03743.  NHI Mechanical Motion DBA New Hampshire Industries was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(2), which was contained in OSHA inspection number 1450115, citation number 1, item number 1 and was affirmed as a final order on February 10, 2020, with respect to a workplace located at 35 Connecticut Riverbend Place, Claremont, NH 03743.
