Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or about October 7, 2021, while replacing roofing shingles on a commercial building located at 370 West Bagley Road, Berea, Ohio 44017, the employer did not ensure employees working from an approximate 8/12 pitch roof were protected from a fall height of approximately 23.4 feet, by utilizing a guardrail system, safety net system and/or personal fall arrest system.
