29 CFR 1904.7(b)(3):The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent: Walmart, Waterloo, IL: On or about October 7, 2021, the employer did not enter correctly the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2021: A. On or about July 31, 2021, a recordable injury occurred when a Frozen Food employee received a back injury from a slip/fall incident. The employee slipped on ice that was on the floor. The employee received medial treatment beyond first aid, but the injury was not classified. B. On or about August 31, 2021, a recordable injury occurred when a Tire Lube employee received multiple body part injuries from being struck by equipment while in a vehicle pit. The injuries resulted in days away from work, but was not classified. C. On or about October 18, 2021, a recordable injury occurred when a Dairy employee received a knee injury after tripping and falling over a pallet. The injury resulted in a restricted duty case, but was not classified. D. On or about November 6, 2021, a recordable injury occurred when a Cashier employee received a knee injury after tripping and falling over a shopping cart. The employee received medial treatment beyond first aid, but the injury was not classified. E. On or about November 10, 2021, a recordable injury occurred when a Cashier Employee received hand injury following a fall while walking to an exterior soda machine. The employee received medial treatment beyond first aid, but the injury was not classified.
