29 CFR 1926.1053(b)(13):The top or step of a stepladder was used as a step: On or about October 7, 2021, at a residential construction, the employer did not ensure employee was not standing at the highest step of an extension ladder when working at approximately six (6) feet above the ground, exposing an employee to fall hazard(s).

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards. On or around October 7, 2021, at a residential construction, the employer did not train an employee to use an extension ladder. An employee installing metal flashings stepped onto the extension ladder's top rung, exposing the employee to fall hazards approximately six (6) feet above the ground.
