29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: (a) On or about October 5, 2021, employees were able to access the point of operation of the saw that was not guarded on the Green Saw Machine, thereby exposing employees to lacerations and amputation hazards. (b) On or about January 26, 2022, employees were able to access the point of operation of the saw that was not guarded on the Yellow Saw Machine, thereby exposing employees to lacerations and amputation hazards. (c) On or about January 26, 2022, employees were able to access the point of operation of the saw that was not guarded on the Ox (Blue) Saw Machine, thereby exposing employees to lacerations and amputation hazards. (d) On or about January 26, 2022, employees were able to access the point of operation of the saw that was not guarded on the Purple Saw Machine, thereby exposing employees to lacerations and amputation hazards. (e) On or about January 26, 2022, employees were able to access the point of operation of the saw that was not guarded on the Red Saw Machine, thereby exposing employees to lacerations and amputation hazards. (f) On or about January 26, 2022, employees were able to access the point of operation of the winder that was not guarded on the Green Saw Machine, thereby exposing employees to lacerations and amputation hazards. 29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) On or about October 5, 2021, at N60W15725 Kohler Lane, Menomonee Falls WI 53051, the employer did not ensure that employees had been properly trained on Hazard Communication, in that employees had limited knowledge on what Hazard Communication was and the hazards associated with chemicals that they use while manufacturing cardboard paper tubes including but not limited to Adhesive 7419 (the glue used to form the tubes).
