An Inspection was not conducted by the competent person prior to the start of the work and as needed throughout the shift. Opposite 200 Hartford Turnpike, Shrewsbury, MA: On or about October 8, 2021, the competent person(s) on the employer's worksite did not conduct inspections as needed throughout the shift for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. As a result, an employee was observed working in an excavation where the protective system was more than 6 feet from the bottom of the excavation where the employee was working.
