(b)	On or about 7 October, 2021, and at times prior, for employees exposed to respiratory hazards where an employee broke concrete with a jackhammer for approximately two hours in absence of a water delivery system or a dust collection system at or near the Mike O'Callaghan Military Medical Center on Nellis AFB, Nevada.  (c)	On or about 7 October, 2021, and at times prior, for employees exposed to respiratory hazards where an employee broke concrete with a skid steer mounted jackhammer for approximately two hours in absence of an enclosed cab or the application of water and/or dust suppressant at or near the Mike O'Callaghan Military Medical Center on Nellis AFB, Nevada.
