29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: Maintenance Facility Building 635: A bench grinder (Missing identification label) had two grinding wheels on the left and right and the work rests openings on both sides were about 1/2 of an inch. Employees were exposed to struck-by/caught-in hazards.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch: Maintenance Facility Building 635: A bench grinder (Missing identification label) tongue guards on both sides were not adjusted to 1/4 of an inch. The tongue guards opening were more than one inch. Employees were exposed to face injuries.
