New residential construction located on Lot #C503 at 125 Carlyle Lane, Fayetteville, GA 30214, Roof: On or about 10/12/2021, the employer exposed employees to fall hazards of 9-feet, 4-inches to the lower level, in that a fall protection system provided was not anchored as intended by the manufacturer as the life-line used was wrapped around a roof ridge and attached to itself and not to the metal anchor provided in the system.
