29 CFR  1926.102(a)(1):Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a) Leonardo Torres Cordero, LLC located at 11210 Juniper Way, Plain City, OH 43064:  On and preceding October 13, 2021, the employer did not ensure that employees wore eye protection while utilizing pneumatic nail guns and a reciprocating saw. Workers were installing framing pieces using pneumatic nail guns powered at 90-120 psi and cutting plywood with an electric circular saw without the use of eye protection. The following condition exposed employees to struck-by hazards to the eyes.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     a) Leonardo Torres Cordero, LLC located at 11210 Juniper Way, Plain City, OH 43064:  On and preceding October 13, 2021, the employer did not ensure that the portable ladder used to gain access to the second floor extended at least three feet beyond the upper landing that they were using for access.  Two 8 foot A-Frame ladders were used to gain access to an upper landing that was approximately 11 feet. The following condition exposed employees to falls up to 11 feet.

29 CFR  1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:   a) Leonardo Torres Cordero, LLC located at 11210 Juniper Way, Plain City, OH 43064:  On and preceding October 13, 2021, the employer did not develop and maintain an effective safety and health program to ensure that hazards were identified and corrected onsite while employees performed framing work.  Leonardo Torres Cordero, LLC  was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1521244, citation number 1 item number 1a and was affirmed as a final order on 3 September 2021, with respect to a workplace located at 11173 Plum Ridge Place, lot 558, Plain City, OH 43064.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Leonardo Torres Cordero, LLC located at 11210 Juniper Way, Plain City, OH 43064:  On and preceding October 13, 2021,  the employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower level were protected from falls. Framers were installing framing pieces on a throughout a new construction residential home at heights that varied between 8 and 12 feet. The employees were not utilizing proper fall protection which exposed them to falls up to 12 feet.  Leonardo Torres Cordero, LLC  was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1521244, citation number 1 item number 2 and was affirmed as a final order on 3 September 2021, with respect to a workplace located at 11173 Plum Ridge Place, lot 558, Plain City, OH 43064.
