29 CFR  1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers.  a) On or about 10/07/2021, at the kitchen area of the site, Williamsville, NY. Employer was the controlling employer of the site that was under construction for a new restaurant. Employer did not cover a hole, which was created by other trades in the kitchen area,  resulting in the carpenters exposed to trip and fall hazards. The hole was 2 feet wide, 29.5 feet long, and 6-8 inches deep.  Carpenters were 7 inches away from the hole working from a step ladder.  NO ABATEMENT DOCUMENTATION REQUIRED
