Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Christiana High School at 190 Salem Church Road Newark, DE 19713: On or about October 13, 2021, the employer failed to ensure employees had and used fall protection while working approximately 15 feet above the grade, exposing employees to fall hazards.

29 CFR 1926.503(a)(1): The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards: a) Christiana High School at 190 Salem Church Road Newark, DE 19713: On or about October 13, 2021, the employer did not train each employee exposed to a fall hazard to properly recognize and minimize fall hazards. Abatement Note: The training program must enable the employees to recognize the hazards of falling and must train each employee in the procedures to be followed in order to minimize these hazards. Each employee must be trained by a competent person in the following: 1. The nature of fall hazards in the work area; 2. The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used; 3. The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; 4. The role of each employee in the safety monitoring system when this system is used; 5. The limitations on the use of mechanical equipment during the performance of roofing work on low-sloped roofs; 6. The correct procedures for the handling and storage of equipment and materials and the erection of overhead protection; 7. The role of employees in fall protection plans; 8. The standards contained in this subpart. The employer must verify compliance with paragraph (a) of this section by preparing a written certification record that will contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.

The ladder used was separating/spidering through the rails reducing the capacity of the ladder. Furthermore, the ladder was not removed from service and tagged with Do Not Use as required by the standard and exposed employees to fall hazards approximately 15 feet above grade.
