29 CFR  1926.502(b)(3):  Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge:  Worksite, 503-509 Communipaw Ave., Jersey City, NJ  07304  The employer directed two employees to perform elevator installation work from the 1st to 5th floor and roof level bulkhead.  During such operations workers were exposed to fall hazards of up to 55 ft. in that  4 x 8 foot plywood used as guardrail barriers were not secured to the elevator shaft openings at each floor and could fall outward or slide away from the  shaft opening.    Violation occurred on or about October 13, 2021

29 CFR  1926.502(d)(11):  Lifelines were not protected against being cut or abraded:  Worksite,  503-509 Communipaw Ave, Jersey City, NJ 07304  The employer directed two employees to perform elevator installation work on the 5th floor with a fall hazard of approximately 55 feet to the shaft bottom.   The two employees used a  Personal Fall Arrest System with a lifeline tied around a cinderblock concrete column at the bulkhead room floor above the 5th floor.  These synthetic rope lifelines were not protected from being cut or abraded by the  cinderblock column corners they were tied around.     Violation occurred on or about October 13, 2021
