29 CFR  1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system  (a) On or about October 14th, 2021, at the job site located at 5121 Cheshire Glenn Rd, Canandaigua, NY, a worker was exposed to fall hazards while installing sheathing material and working near roof edges of a residential structure.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:  (a) On or about October 14th, 2021, at the job site located at 5121 Cheshire Glenn Rd, Canandaigua, NY, a worker was exposed to 9' fall hazards while utilizing a 16' Werner extension ladder extended 2 feet above overhead trusses.
