29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, and flying chips: a) Machine Shop; on or about October 14, 2021 and at times prior to, operators of the Clausing lathe were exposed to amputation hazards, in that the employer did not install a guard to prevent accidental contact with the 12 in. diameter chuck. b) Machine Shop; on or about October 14, 2021 and at times prior to, operators of the Haas VF-5 Machining Center CNC Mill were exposed to amputation hazards, in that the employer did not ensure that the doors were protected by interlocks to prevent accidental contact with the point of operation. c) Machine Shop; on or about October 14, 2021 and at times prior to, operators of the Haas VF-2 Machining Center CNC Mill were exposed to amputation hazards, in that the employer did not ensure that the doors were protected by interlocks to prevent accidental contact with the point of operation.
