29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) 64 Maple St. Rouse Point, NY-On or about October 08, 2021, employees were removing debris from the roof of the facility operations without the use of any fall protection to include fall arrest, restraint or a monitoring system fall protection, exposing employees to a fall hazard of approximately 22 feet.

29 CFR  1926.503(b)(1):The employer did not verify fall protection training by preparing a written certification record.  a) 64 Maple St. Rouse Point, NY-On or about October 08, 2021, The employer failed to document fall protection training with written certification for employees exposed to fall hazards while removing debris from the roof of the facility. Employees were exposed to a fall hazard of approximately 22 feet to the next lower level.
