On or about October 14, 2021, during the roof sealing project at Fun 'N Stuff located at 661 Highland Road, Macedonia, OH 44056; ; the employer did not ensure employees working on the roof were protected from a fall of approximately 14 feet by utilizing a personal fall arrest system or any other form of fall protection system.

On or about October 14, 2021, during the roof sealing project at Fun 'N Stuff located at 661 Highland Road, Macedonia, OH 44056; the employer did not ensure employees working from a GROVE man-lift model AMZ51XT aerial lift were wearing and utilizing a body belt (harness) and lanyard attached to the basket of the aerial lift.
