29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):  a. On or about October 8, 2021, employees were exposed to machine hazards associated with moving parts during servicing and cleaning of the Line 2 Pizza Topping Machine (Serial #: 99TS381). The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a. On or about October 13, 2021, in the Pizza Topping Department, the employer did not ensure that the in-going nip points on the conveyor belt of the Line 2 Pizza Topping Machine (Serial #: 99TS381) were guarded.

29 CFR  1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.   a. On or before October 8, 2021, the employer did not ensure that an employee who was required to clean the Line 2 Pizza Topping machine had been properly trained on the process of deenergizing and applying energy control devices to the equipment.  General Mills was previously cited for a Serious violation of this Occupational Safety and Health Standard 1910.147(c)(7)(i)(A), which was contained in OSHA inspection number 1311546, citation number 1, item number  1b and was affirmed as a final order on June 12, 2018, with respect to a workplace located at 1 Red Devil Rd, Hannibal, MO 63401.
