OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards:  a)  The Flooring Place Inc, Appliance Warehouse Storage Racking:  On or about October 14, 2021 and at times prior, employees were exposed to struck by hazards while loading and unloading material from the storage racks that were not securely bolted to the floor.  b)  The Flooring Place Inc, Lux Main Warehouse Storage Racking:  On or about October 14, 2021 and at times prior, employees were exposed to struck by hazards while loading and unloading material from the storage racks that were not securely bolted to the floor.  c)  The Flooring Place Inc, High Dock Warehouse Storage Racking:  On or about October 14, 2021 and at times prior, employees were exposed to struck by hazards while loading and unloading material from the storage racks that were not securely bolted to the floor.  d)  The Flooring Place Inc, Main Warehouse Storage Racking:  On or about October 14, 2021 and at times prior, employees were exposed to struck by hazards while loading and unloading material from the storage racks that were not securely bolted to the floor.  e)  The Flooring Place Inc, 16 & 17 Warehouse Storage Racking:  On or about October 14, 2021 and at times prior, employees were exposed to struck by hazards while loading and unloading material from the storage racks that were not securely bolted to the floor.  f)  The Flooring Place Inc, Appliance Warehouse Storage Racking:  On or about October 14, 2021 and at times prior, employees were exposed to struck by  hazards while loading and unloading material from storage racks that did not display a plaque designating load sizes.  The employer failed to identify the load rating of all the storage racks in the Appliance Warehouse to ensure they were not overloaded.  g)  The Flooring Place Inc, Lux Main Warehouse Storage Racking:  On or about October 14, 2021 and at times prior, employees were exposed to struck by hazards while working in and around the steel storage rack that did not display a plaque designating load sizes.  The employer failed to identify the load rating of all the storage racks in the Lux Main Warehouse to ensure they were not overloaded.  h)  The Flooring Place Inc, High Dock Warehouse Storage Racking:  On or about October 14, 2021 and at times prior, employees were exposed to struck by hazards while working in and around the steel storage rack that did not display a plaque designating load sizes.  The employer failed to identify the load rating of all the storage racks in the High Dock Warehouse to ensure they were not overloaded.  i)  The Flooring Place Inc, Main Warehouse Storage Racking:  On or about October 14, 2021 and at times prior, employees were exposed to struck by hazards while working in and around the steel storage rack that did not display a plaque designating load sizes.  The employer failed to identify the load rating of all the storage racks in the Main Warehouse to ensure they were not overloaded.  j)  The Flooring Place Inc, 16 & 17 Warehouse Storage Racking:  On or about October 14, 2021 and at times prior, employees were exposed to struck by hazards while working in and around the steel storage rack that did not display a plaque designating load sizes.  The employer failed to identify the load rating of all the storage racks in the 16 & 17 Warehouse to ensure they were not overloaded.
