Workers were installing framing pieces using pneumatic nail guns powered at 90-120 psi without the use of eye protection. The following condition exposed employees to struck-by hazards to the eyes.

Framers were installing framing pieces on a throughout a new construction residential home at heights that varied between 12 and 25 feet. The employees were not utilizing any form of fall protection which exposed them to falls up to 25 feet.

29 CFR 1926.1053(b)(1): Ladder extension was as such that ladder deflection under a load, did by itself, cause the ladder to slip off its support: a) David Salto located at 6824 Johnston Rd, Dublin, OH 43017: On and preceding October 14, 2021, the employer did not ensure that the ladder used to gain access to the second floor extended at least three feet beyond the upper landing that they were using for access. A 12 foot wooden job-made ladder was used to gain access to an upper landing that was approximately 12 feet without the ladder secured. The following condition exposed employees to falls up to 12 feet.
