29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: Employees engaged in general maintenance activities at the mechanical room outside of the main building were exposed to electrical shock hazards in that the electrical box cover for the furnace was loosing, not fully engaged, exposing energized wires. There was no protection from contacting with the open energized wires inside the junction box.

29 CFR 1910.305(g)(1)(iv)(A):Flexible cords and cables were used as a substitute for fixed wiring of a structure: Employees engaged in care taker and maintenance activities at the main building, by the relaxing room next to the PPE storage area were exposed to fire and electrical shock hazards in that the orange flexible cord used to supply power to the electric bed was used in lieu of fixed wiring. A flexible cord was used as a substitute for fixed wiring to energize the electric bed.
