29 CFR  1926.451(b)(1)(i):Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  Location: 2064 Royce Street, Brooklyn, NY  On or about 10/15/2021  a) Employee worked from a scaffold platform that was not fully planked. The working platform had two planks and the space between the planks was more than two feet.

Cross braces were used as a means of access.:  Location: 2064 Royce Street, Brooklyn, NY; front of building  On or about 10/15/2021  a) A 3-tier high supported scaffold had no safe means of access.  An employee climbed the scaffold frames and cross braces to access the working platform.

Note:  For voltage less than 300 volts the minimum distance is 3 feet.  Location: 2062 Royce Street, Brooklyn, NY  On or about 10/15/2021  a) A 3-frame high supported scaffold was erected less than 3 feet away from the power line.  The service voltage of the power line was 110 volts.
