29 CFR 1926.451(f)(7):Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person. a. On or about October 15, 2021, at the above addressed jobsite, employees were working on/ from a cantilevered section of platform, which was supported by and attached to a supported scaffold by outriggers, during which time the scaffold and scaffold components had not been erected, moved, dismantled and altered by trained and experienced employees, nor while under the supervision and direction of a Competent person nor while under the supervision and direction of a Qualified person in scaffold erection, movement, dismantlement or alteration, and wherein the scaffold system was observed to have several visible hazards such as cantilevered work platforms, eccentric loads and missing guardrails along the second level/ tier platforms; therefore exposing the employees to a fall hazard.

29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length, extended over its support more than 12 inches: a. On or about October 15, 2021, at the above addressed jobsite, employees were working on/ from a cantilevered section of platform, that which was supported by and attached to a supported scaffold by outriggers, during which time the outriggers/ cantilever section of platform extended over/ past the scaffold upright supports by approximately 22 inches/ more than 12 inches, and wherein the outriggers/ cantilevered platform section was not designed/ intended to be used as a cantilever, nor was the cantilevered section of platform installed so as to ensure that the eccentric loads of the cantilevered section of platform was able to support employees and/ or cinder blocks/ materials without tipping; therefore exposing the employees to a fall hazard.

On or about October 15, 2021, at the above addressed jobsite, employees were working on/ from a cantilevered section of platform, that which was supported by and attached to a supported scaffold by outriggers, during which time the outriggers/ cantilever section of platform extended over/ past the scaffold upright supports by approximately 22 inches/ more than 12 inches, and wherein the outriggers/ cantilevered platform section was not designed/ intended to be used as a cantilever, nor was the cantilevered section of platform installed so as to ensure that the eccentric loads of the cantilevered section of platform was able to support employees and/ or cinder blocks/ materials without tipping; therefore exposing the employees to a fall hazard.
