29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  Jobsite:  On or  about September 22, 2021, two employees were installing trusses on a new residential home and they were not protected from falling to the ground.  The fall hazard was up to approximately 30 feet.

29 CFR  1926.502(i)(3):All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees:  (a)  Jobsite:  On or about September 24, 2021, plywood was placed over the front porch area and the stairs area on the first floor and none of these pieces of plywood were secured in place.  The fall hazard to employees was about 10 feet 6 inches to the concrete basement floor.

29 CFR  1926.502(i)(4):  All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard:  (a)  Jobsite:  On or about September 24, 2021, none of the pieces of plywood that were over the front porch area and the stairs to the basement were marked as "hole" nor "cover" nor were any of the covers color coded.  The fall hazard to the employees was approximately  10 feet 6 inches to the basement  concrete floor.
