29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Northern Extrusion Tooling, Inc. at 905 West 19th Street, Yankton, SD 57078: on or about and at times prior to October 15, 2021 the employer did not ensure that machine specific, step-by-step procedures were developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section, including but not limited to the ovens within the heat-treat area. This condition exposed employees to hazards associated with potential hazards from gas lines not being locked out.

29 CFR 1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a)Northern Extrusion Tooling, Inc. at 905 West 19th Street, Yankton, SD 57078: On or about and at times prior to October 15, 2021, the employer did not ensure that electrical equipment was used according to the listing or labeling instructions. A Relocatable Power Tap (surge protector) was being used to power a DeWalt Chop saw and additional equipment within the woodworking / pallet building area. This condition exposed employees to electrical shock and fire hazards.
