29 CFR 1926.651(j)(2):Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: a) Jobsite, 2604 Jackson Avenue, Windber, PA: On or about October 7, 2021 an employee went into a trench approximately 4 foot in depth with the spoil pile only 1 foot away from the edge to make the connection from the new water line to the curb box/city tap at the end of the trench at the street.
